Having a baby? Thinking about transferring care? Want to learn more about our providers and tour the office?
Please call our office and see if we are accepting new patients and participate with your insurance.
We can schedule a visit called a Meet and Greet to meet with you to answer questions, see how our office works, and meet one of our providers. We are also happy to talk with you by phone if you prefer. We want to help you make the best decision for your family.
To do this successfully, we must be able to review all of your medical records from all providers that you have seen. It is our policy to receive your previous medical records before we can scheduled a well child visit.
For Newborns
Bringing a newborn to a new pediatric practice can be a prudent decision to ensure comprehensive care and support for your child's lifelong health. It allows for establishing a rapport with the healthcare providers early on, fostering a sense of familiarity and trust crucial for ongoing care.
For New Families Transferring To Us
When transferring a child to a new pediatric practice, it's essential to gather and organize all medical records, including vaccination records and any relevant test results. Communicate with both the current and new pediatric practices to ensure a smooth transition of care, providing necessary information about the child's medical history and ongoing treatments.
For patients transferring to our practice, we will need to have received all of the medical records prior to scheduling a Well Child Visit. If you child is ill, we can schedule a sick visit without previous medical records. Medical records give vital information on your child's previous illnesses, medical conditions, growth, and immunization records.
Please be aware that under federal (HIPAA) and state laws, your previous medical provider may charge a "reasonable, cost-based fee" to copy your children's medical records. This cost is your responsibility. This fee may be waived if records are sent directly to our office.
If you have not already done so, please sign a release of information form and provide it to your children's previous medical provider. We are happy to fax the release of information form to the provider's office for you.
