Insurance & Billing

We accept most major insurance carriers.  Please have your insurance card with you at the time of your visit.  If you have new insurance and have not yet received your insurance card, you will need the following information: insurance company name, claim address, co-payment amount, policy holder name, policy number, group number, and insurance provider phone number.

Please verify that we are a provider for your insurance carrier prior to your appointment.

We request that all co-payments be paid at the time of service. 

We accept cash, checks, credit and debit cards.
